Waldron Health and Rehab Center is a 79-bed skilled nursing facility with a staff of 100 full- and part-time professionals meeting the
needs of our community and those requiring nursing and rehabilitation services. We are proud of the dedicated and caring staff
members; many with 15+ years of longevity and dedication to Waldron Health and Rehab Center.
We offer physical rehabilitation, outpatient therapy, and a stroke rehabilitation program. We have a restorative team, a full-time wound care nurse, as well as on site services for lab and x-ray services. Waldron also has a consulting psychiatrist, podiatry, optometry, audiology, and dental services offered in the facility for the convenience of our residents.
231 patients were successfully rehabbed to home from January 2013 to August 2014!

Waldron Health and Rehab Center has received a high ranking of five stars overall in U.S. News & World Report’s 2011 Best Nursing Homes, available online at www.usnews.com/nursinghomes.
The rankings highlight facilities across the nation providing high-quality care and are a worthwhile tool for millions of elderly Americans and their families in search of a nursing home. U.S. News’s Best Nursing Homes profiles more than 15,000 facilities and ranks them by state, using data and quality rankings from the federal government. The rankings are updated quarterly.
The rankings rely on information from the Centers for Medicare and Medicaid Services, the federal agency that sets and enforces quality standards for all homes enrolled in Medicare or Medicaid. Homes receive an overall rating of one to five stars from the government based on the number of stars earned in three categories: health inspections, nurse staffing and quality of care.
Facilities that ranked highest in Best Nursing Homes 2011 earned overall five-star ratings in all three categories. Only about one home in seven met this standard.
Mr. Donald L. came to us after a brief stay in the hospital. He needed extensive assistance with walking, transfers, bathing, dressing, and most aspects of activities of daily living.
Our team of dedicated physical therapists and occupational therapists worked with him on pulmonary rehabilitation, energy conservation, general aerobic re-conditioning, activities of daily living tasks, self care skills, and mobility training for several weeks.
It is our pleasure to announce that Mr. Donald L. has returned home with his wife. Thanks to his hard work and our team’s dedication to assist him, his return home has been successful.
Mr. George M. came to Waldron Health and Rehab center with difficulty in walking, inability to take care of himself, and difficulty maintaining oxygen saturation and difficulty in breathing. Our intensive and individually tailored therapy program worked with Mr. George M. to get stronger, better, and more independent with his day to day care.
Today, Mr. George M. has successfully returned home with his wife, staying there and getting even stronger. Mr. Moore shared his experience with our team:

Mrs. Joanne L. came to us after she had a fracture in her right hip that was surgically fixed. Mrs. Joanne L. needed extensive assistance with walking, transferring, dressing, bathing, and other activities of daily living. Moreover, the pain limited her from standing for more than a minute.
Our dedicated team of therapists and nursing staff worked together with the physician and Mrs. Joanne L. for about a month. Before long she was taking care of her needs and walking using a walker all over the building.
Mrs. Joanne L. has returned home with her husband and returned for continued outpatient therapy.
Mrs. Julia R. came to us after a brief stay in the hospital. She needed extensive assistance with walking, transfers, bathing, dressing, and most aspects of activities of daily living.

Admitted to Fairview Skilled Nursing and Rehab Center on March 22, 2018 from Toledo Hospital. Prior to arriving to Fairview, Mr. C. was living at home with his spouse. He was not able to complete bed mobility, sit to stand transfers or simple hygiene tasks without assistance.
With the help of our Therapy Department (Affirma), Mr. C. completes transfers with minimal assistance, he is able to complete hygiene and grooming tasks with verbal cues to initiate.
Noble discharged from Fairview on April 30th, 2018 to home with his spouse.
