Our nursing and rehab services assist individuals with regaining their highest level of independence, improve their current abilities, and making their stay as comfortable as possible. We provide residents with customized care and attend to their specific needs. We’ve made it our goal to transform typical health services into HEALTHCARE WITH HEART!

He has stayed at many of the other local facilities, and in my opinion, Palo Alto Sub-Acute is hands down the best facility. They have a very competent administrative staff, and the nurses and rehabilitation staff are caring and attentive. My father has been bed ridden for over a month, and Palo Alto Sub-Acute had him standing and well on his way to recovery in less than one week. Normally my father would be complaining to come home by now. This facility has him wanting to stay longer and get stronger. The facility is located on a very private residential street a few blocks from downtown Palo Alto. His room gets lots of sunshine, and it has direct access to an internal courtyard. “My grandmother had recently fractured her hip so she needed somewhere to get physical therapy. I had a friend recommend this facility for her. At first my grandmother did not want to attend, as she is skeptical of “nursing homes”. I reassured her that everything would be fine. As it turned out I was right! These three weeks were enjoyable for her ! This place has such a caring, honest, and friendly staff as well as professional. Any questions I had were answered when needed! They care about their patients. Physical therapy was well done! The amount of effort the staff puts into their patients makes them feel welcomed and cared for. The facility itself is clean and in addition the rooms had their own access to the patios where families and patients can spend time together when the weather is nice. “My 93 year old partner was an arrival here on Monday, June 22 and is being well cared for. He is getting very good PT and is showing improvement. It is walking distance from home and I visit several times per day. The staff is exceptionally nice and professional. While my foot was in the cast/boot, the therapy focused on getting my legs stronger while in the wheelchair and my exercises did strengthen my legs and my upper body and arms. After the cast was removed, the therapy included standing and walking. I can now walk with assistance of walker and have regained my mobility.”

Grace needed to be fully reconditioned. She was in ICU for 2 months due to complications from successful vascular by-pass. During the surgery, she had an embolism in her lung. Grace was intubated twice and finally a tracheotomy was done.
A local hospital successfully de-cannulated Grace and recommended Palo Alto Sub-Acute & Rehabilitation Center. She needed PT and OT services since she was in bed for over three months.
Grace can now walk with a walker, put her blouse on, unbutton buttons, write, sit up in bed, and feed herself. She is much stronger in her legs, upper body, and arms.
