Grant Cuesta Sub-Acute and Rehabilitation Center, located in Mountain View near San Jose, CA. in the Bay Area, offers 24-hour skilled nursing and rehabilitation therapy services with experienced staff dedicated to treating our residents with dignity and respect. Our nursing and rehab care assist individuals in regaining their highest level of independence, improve their current abilities, and making their stay as comfortable as possible. Our nursing home offers everything from physical therapy, occupational therapy, post-acute care to Alzheimer’s care. We provide residents with a customized program and attend to their specific needs. As part of the Covenant Care family, we’ve made it our goal to transform typical health services into HEALTHCARE WITH HEART!

“We chose to put my 84-year-old mother here because she just had open-heart surgery and it is located very close to my house. I visit her daily and I can tell you that I am very pleased with the attention and care that she has received. The doctors and nurses are great and their care has been excellent with my mother. She has made an incredible recovery in the three weeks that she has been there. The caregivers have all been loving and very nice to my mother. She always has a bath, gets her hair washed and looks really nice every single time I come to see her. They fix her hair; dress her, help her with her perfume and hand cream too. My mother has been very fortunate to be in a room with a very lovely woman who has the nicest family! They do things for my mother and we do things for their mom. We could never have gotten a better place for my mom! She is very happy there and this is what we wanted for her. As I said, her recovery from her open-heart surgery has been remarkable and her cardiologist from Stanford is so pleased with her recovery too. All of her therapists have been extremely great with my mom and we feel that this has a great deal to do with her speedy recovery. We feel very lucky to have had such great care for my mother. All the staff has been outstanding so I can’t imagine a better place for my mother. . Now that my mom is feeling better she went to one of the activities, which she enjoyed very much. In the activity room they were making cookies so we got to eat some. Yum, yum! The staff member was so friendly, kind and inclusive. We had a very enjoyable time there. Taking my mom to her appointments in the rehab van has been a very nice experience for our family. My dad and I have gone along on her visits to the doctor and we had very nice outings with my mom. The driver was always very punctual and friendly so we could not ask for better service. I highly recommend the center for your loved one. We could not have found a better rehab for ours!”
