Covenant Care provides post-acute services at our facilities that are an integral part of a patient’s continuum of care.
Covenant Care is a post-acute provider of services, mainly Skilled Nursing Facilities, Home Health and outpatient therapy. Skilled Nursing is a licensure, and post-acute describes an array of services downstream from an Acute Care Hospital. Facilities that participate in either Medicare or Medicaid must meet the National Fire Protection Association Life Safety Code and federal regulations for long-term care facilities.
Covenant Care is a solution for high quality and efficient post-acute healthcare.
Our facilities are interconnected through information services that enable facilities to communicate with each other and receive information timely with which to best care for their patients. Our goal as a company is to provide each facility with the information it needs to provide the highest level of care possible.
All facilities must meet state licensure requirements, and some facilities may be accredited to provide subacute medical or rehabilitative care.
Some of the specialty services and programs we offer include:
- Orthopedic-Fracture Recovery

Sub-Acute Medical Care
A subacute facility is typically a description for extensive clinical care and/or ventilator care. Covenant Care has two licensed subacute facilities.
Personal care is provided to residents who need help with various activities. Such assistance most often relates to performance of activities of daily living (ADLs), which includes eating, bathing, dressing, toileting, transferring, personal hygiene. Certified nurse assistants provide many of these services.
Covenant Care has 4 facilities that provide Residential Services. Each facility provides general supervision services, such as room and board, and a planned program for the social and spiritual needs of the resident.
