Covenant Care provides post-acute services at our facilities that are an integral part of a patient’s continuum of care.

Post-Acute and Skilled Nursing Facilities (SNFs) both provide the same services with increased emphasis on rehabilitative therapies for convalescent patients. SNFs are eligible to participate in the Medicare and the Medicaid programs. Facilities that participate in either Medicare or Medicaid must meet the National Fire Protection Association Life Safety Code and federal regulations for long-term care facilities.

At Covenant Care, we have developed services that meet the needs of an increasingly demanding industry. Our facilities are interconnected through information services that enable facilities to communicate with each other and receive information timely with which to best care for their patients. Our goal as a company is to provide each facility with the information it needs to provide the highest level of care possible.

All facilities must meet state licensure requirements, and some facilities may be accredited to provide subacute medical or rehabilitative care.

There are 4 basic types of services are offered by our facilities:

Nursing & Rehabilitative Care

All nursing facilities require the professional skills for a registered or licensed practical nurse.

Nursing services include assessment, treatments, injections, coordination of care, and medication administration.

Rehabilitative services such as post-hospital stroke, heart or orthopedic care are available in addition to related services such as respiratory therapy, physical therapy, occupational therapy, or speech therapy.

Dental services, dietary consultation, laboratory, X-ray, and pharmaceutical services are also available.

Sub-Acute Medical Care

Residents in nursing facilities are under the care of physicians, who visit regularly and are responsible for the residents’ overall plan of care.

Physicians certify the need for nursing care and may serve as a resource for information about post-acute care facilities in the community. When individuals enter the facility, physicians write orders for necessary medication and play a role in the development of the residents’ care plan, including restorative and rehabilitative procedures, special diets and treatments. Every nursing facility has a contracted physician on call to handle emergencies.

Personal Care

Personal care is provided to residents who need help with various activities. Such assistance most often relates to performance of activities of daily living (ADLs), which includes eating, bathing, dressing, toileting, transferring, personal hygiene. Certified nurse assistants provide many of these services.

Residential Services

Covenant Care has 4 facilities that provide Residential Services. Each facility provides general supervision services, such as room and board, and a planned program for the social and spiritual needs of the resident.
