Our physical, occupational, and speech therapists work with residents confronting a variety of conditions, from stroke to dysphasia to Parkinson’s disease, as well as conditions resulting from elective surgeries, multiple trauma, recovery from amputations. Our in-house rehabilitation team creates an individualized treatment program developed with the interdisciplinary team of nursing, social services, activities and dietary in consultation with your physician. Physical Therapy focuses on identification, prevention and rehabilitation of individuals with physical impairment and pain. Following evaluation, the physical therapist develops an individualized treatment program addressing the areas of lower extremity strength, balance, coordination, functional movement, ambulation, endurance and general mobility. The goal is to achieve safety in a variety of areas including:

Occupational Therapy addresses activities of daily living skills. The Occupational Therapist assists the patients in achieving their highest level of functioning, and in maximizing independence by focusing on:

  • Home Assessments and Modifications

Speech/Language Therapists provide a continuum of services including assessment, education and treatment of patients with communication, cognition and swallowing problems. Treatment programs are directed towards improving communication skills in the area of comprehension and expression. Patients admitted to the facility that are receiving nutrition partially or exclusively via a gastrostomy tube are evaluated for their potential to return to solid foods as appropriate. Speech therapists work to restore abilities in:

Additional Services

Complex Wound Care
Designed for residents recovering from serious wound complications or wound related surgery, our Wound Care Program offers individualized wound care as well as comprehensive wound management. Some of our services may include: the development of a interdisciplinary care plan by a dedicated wound care physician that addresses nursing, dietary and rehabilitation treatment regimes; specialized mattresses along with post-surgical recovery. Our goals are to focus on the health of our residents, to prevent wound-related complications, and to provide effective quality wound care.

Nutrition Services
The dining experience is an important part of a resident’s medical and social well-being. Therefore, meeting the nutritional needs of our residents with well-balanced meals and snacks is just one part of a resident’s overall plan of care. Our team of nutrition professionals includes a Nutrition Services Supervisor who manages the foodservice operations and a Registered Dietitian who evaluates the nutrition needs of our residents. They work together to meet the goal of the Nutrition Services Department to prepare meals that taste great, as well as meet the special dietary needs of our residents.

Outpatient Services
Our outpatient services include physical, occupational, and speech therapy. Residents benefit from the collaborative effort of a team of highly skilled therapy clinicians. We treat a variety of resident conditions including, but not limited to orthopedic, neurological, degenerative and communication disorders. We provide free transportation to and from your appointments with our facility bus.

Transitional Living Unit
Wright Nursing and Rehabilitation Center now offers a Transitional Living Unit. This unit is an evaluation process for rehabilitation patients who are preparing for discharge from the facility. When our residents arrive they get “the full experience” of care offered by our nursing center.

The Transitional Living Unit is a comfortable apartment-like setting within the existing facility, but is furnished complete with a small refrigerator, flat- screen television, as well as a non-hospital bed, private bathroom, and a personal table and chairs.

Transitional Medicare residents are eligible for up to a three day stay. During this time, they will function with little assistance from staff. Our goal is to prepare each resident for a safe transition to their home.

At Wright Nursing and Rehabilitation Center, we understand that the quality of the time you spend with your loved one decreases as needs and demands precipitated by Alzheimer’s increases.

As your loved one’s restlessness begins to create safety risks in your home, your ability to rest is hindered, as well. The Memory Care Unit provides a environment devoted to the health and safety of your loved one, helping to ensure that the time you spend together is as free from stress and as satisfying as possible.

At the Memory Disorder Unit, your loved one receives the highest quality specialized care from an interdisciplinary team of professionals specifically educated to understand and meet the individual needs of each Alzheimer’s Resident – psycho-social, emotional, spiritual, physical, nutritional and medical.

Independence is stressed and encouraged at the Memory Care Unit. Residents are carefully monitored to determine their abilities as their disease progresses. They are allowed to function independently as long as possible and they are assisted with various functions only when they are no longer able to perform them on their own.

While ensuring an active and independent life for our residents as possible, the Memory Disorder Unit provides the following services, tailored to the specific needs of each Alzheimer’s Resident:
