Getting Home

Our overriding goals is to return you to your highest level of function. Our personalized treatment plans are designed to maximize the benefits of our in-house specialized skilled providers during your stay. But we also incorporate programs designed to help you transition from our inpatient residential facility to your own home in a step-wise, safe manner.

Our goal is to make you successful! We want you to return home and stay home!

 

Here is a glimpse of some of our programs that help transition our patients home:

 

Homeward Bound - Out-patient Therapy

Your best chance for successful rehabilitation starts here. Our goal is to restore you to your highest level of independent function. 

Our Homeward Bound program offers outpatient programs 7 days a week that cover a variety of therapies, including physical, occupational, and speech. 

Ares of focus of include, but are not limited to:

  • Cardiology 
  • Orthopedics (pre- and postsurgical) 
  • Pulmonary 
  • Pain management
  • Urinary incontinence 
  • Stroke recovery
  • Balance/Falls

VIP Program

The Validation in Progress (VIP) program last phase of our Homeward Bound therapy program. It is a personalized, team approach to implement a less restrictive environment to replicate the setting you will return to at home. 

This program is designed to make certain that residents transition from our facility to their homes successfully. Its purpose is to ensure you are ready to safely and successfully return home. 

By being able to test their strength and stamina of their newly regained skills, patients can return home with increased confidence. This validation phase also allows for a thorough evaluation to test activities of daily living necessary for independent living. 

In addition to building confidence with our residents, we strongly encourage family involvement during this process. This will increase their awareness and confidence in the capabilities of their loved one. 

This process is an opportunity to identify any need for assistive devices to be used in the home. The team can educate our residents and their families on a proper use of the device to ensure success and reduce the risk of a return stay at the hospital or rehab facility. 

This is the final phase of your stay with us and it is specifically designed to identify any areas of concern or problems that might affect your ability to return home. Potential problems, once identified, can be resolved prior to your discharge, ensuring that you or your loved one are confident about your ability to return home safely. 

During VIP: 

  • You will be asked to do tasks that you will need to be able to do on your own at home, such as access your medications, make your own bed, prepare light meals, etc.
  • The frequency of your therapy sessions will be slowly decreases to match the anticipated level of activity after discharge.
  • Our staff will work with you to make all the necessary arrangements for your return home, including ordering any necessary equipment, arranging home health care, and/or scheduling your out-patient therapy.
