We at Shoreline Care Center have discharged an incredible number of residents back into our community as a result of our strong Nursing and progressive therapy programs. Our success rate in therapy is the result of having onsite therapists who use state of the art modality equipment.

our Therapy Department is managed by an onsite Facility Rehab Director who coordinates skilled services in Physical Therapy, Occupational Therapy and Speech Therapy provided by a chosen team, which consists of licensed therapists, assistants, and aides who join together to offer the best of quality therapeutic services.

This team is not contracted out, they work for our facility, in our facility which adds to the family feeling of our facility.

Our common goal is to work with the patient and the family to ensure a smooth transition to the home, residential care or long term care environment. Special attention is paid to achieving an optimal outcome by establishing individualized treatment plans for each patient. Our overall focus is to promote each patient's quality of life and assist them in achieving their highest level of independence and safety.

We methodically review and monitor each resident's progress in therapy making sure that the specified objective is met or will be eventually met. A case summary is kept and available upon request for the family.
