Our Homeward Bound programs are intensive rehabilitation services specifically designed for the purpose of safely returning residents back to their former living arrangements in the community.
The Homeward Bound programs provide ongoing evaluation and treatment for those who want to increase their functional levels in:
- ADLs (Activities of Daily Living) including feeding, toileting, dressing, mobility, transfers, urinary incontinence, and more;
- Safe ambulating on various surfaces throughout the home and community;
- Increasing strength and endurance to be able to manage independently or at a lower level of assistance.

- Wound management, respiratory health, contracture management, stroke recovery, orthopedic-fracture recovery and much more.
PROGRAM HIGHLIGHTS:
Orthopedic and Fracture Recovery Program Components include: Pain management, skin/incision care, nutritional assessment, positioning, gait and transfer training, ambulation over various surfaces, restoration of activities of daily living and home evaluation to determine if special equipment is needed at the time of discharge.
Pulmonary Program Components include: exercise training of the upper and lower body plus ventilitory muscle training, respiratory therapist supervision, energy conservation, psychological support, educational programs, and home evaluation to determine if special equipment is needed at the time of discharge.
Stroke Recovery Program Components include: safe swallowing, speech retraining, ambulation and gait training on multiple surfaces, transfers and mobility, use of adaptive equipment, and home evaluation to determine if special equipment is needed at time of discharge.
Wound Care Program Components include: consultation with general surgeon and wound specialist, advanced wound care products, wound vac therapy, electrotherapy, ultrasound, short wave diathermy, sharp wound debridement, hydrotherapy, positioning techniques and devices, and nutritional assessment.
