Admissions

Residents can be admitted to Valle Vista Convalescent Hospital 24 hours a day, 7 days a week. Please contact Sonia Halog, in admissions for more information.

At the time of admission, a written agreement shall be executed between the family and resident or responsible party (Power of Attorney). Resident's rights and responsibilities are reviewed, as well as an explanation of specific procedures that need to be followed at the facility.

We at Valle Vista Convalescent Hospital recognize that this admission process, from it's initial decision to final transfer and signing of forms, can be very traumatic for the resident as well as for the family members. We are here to give support as needed, answer questions, and give guidance during the transition period.




Following the initial assessment by nursing, other professionals from other departments (e.g. dietary, therapeutic recreation, social services) will visit the resident and family. An individualized plan of care will be formulated for the resident to assist in recognizing problems and setting goals; with approaches designed to meet the individual needs. Residents, individual care plans are reviewed every quarter. Family members are invited and encouraged to attend these meetings.



Upon Admission:

Resident's are encourage to wear "street clothing" whenever possible. Please bring the following items on admission:

  • 7 changes of clothing
  • Underwear
  • Outerwear (washable)
  • White cotton socks
  • Hosiery
  • 1 pair of comfortable shoes
  • 2 pairs of clippers (closed heel)
  • Nightclothes and robe
  • Sweater

To ensure the safety of our residents, certain items cannot be kept at the bedside or brought in for use. They are as follows:

  • Medications, including over the counter drugs
  • Medication ointments
  • All aerosol spray cans, including deodorant and hairspray
  • Any product labeled "harmful if swallowed," including nail polish remover
  • Extension cords, knives, scissors, and other sharp implements

All food items brought into the facility should conform to the residents diet and must be stored in airtight containers.



CLOTHES MARKING PROCEDURES

All clothes must be clearly marked with the resident's name. Names can be written on the inside collar of T-shirts, blouses, dresses, nightclothes, and on the inside waistbands of pants, skirts, shorts, and slacks. Marking pens can be obtained from the social worker, admissions coordinator, or nursing staff on the unit for labeling clothes.

It is our policy: You check in the resident's clothing upon admission and make sure residents inventory of items form is completed upon admission signed and turned into the nursing staff to be added to the resident file. Additional items brought in after admission needs to be added to the inventory form as well. Please be sure those items are clearly marked before leaving it in the resident's room. When any personal items or clothing are removed permanently, notify the nurse in charge so that the inventory list can be updated.

The family may elect to do the resident's personal laundry or the facility can do it at no additional charge. Residents under certain State and Federal programs are provided laundry service.
