Plans for discharging a resident home begins the day of admission!
Silver Ridge Healthcare Center provides the finest level of rehabilitation services consistent with the philosophy of meeting the needs of those we serve. Under the direction of a physiatrist (a physician who specializes in physical medicine and rehabilitation), we provide individualized treatment programs which allow residents to reach their maximum potential. Family involvement in the rehabilitation process is encouraged. We believe that family support and education are vital aspects in providing excellence in care and assisting the resident to maximize functional ability and independence.
Physical Therapy Services focus on identification, prevention and rehabilitation of individuals with physical impairment and pain. Following evaluation, the Physical Therapist develops an individualized treatment program addressing the areas of lower extremity strength, balance, coordination, functional movement, ambulation, endurance and general mobility.
Occupational Therapy Services address activities of daily living skills, including eating, dressing, hygiene and home management tasks, as well as upper extremity functioning, cognition and perception. The Occupational Therapist assists the residents in achieving their highest level of functioning, and in maximizing independence.
Speech/Language Pathology provides a continuum of services including assessment, education and treatment of residents with communication, cognition and swallowing problems. Treatment programs are directed towards improving communication skills in the area of comprehension and expression. Residents who are admitted to the facility and receiving nutrition partially or exclusively via a gastrostomy tube are evaluated for their potential to return to solid foods as appropriate.
Cardiopulmonary Program incorporates compensatory techniques, breathing training, energy conservation, and diagnostic testing with therapeutic exercise and activities of daily living. Chronic lung disease affects over 30 million Americans and accounts for over 250,000 annual admissions to skilled nursing facilities. This program is a covered service by Medicare Part A, Part B, and private insurance. Patient care plans typically incorporate 2-3 hours of therapy per day for 4-6 weeks. In addition, Silver Ridge Healthcare Center is proud to introduce our first Pulmonary Rehabilitation Program. The primary mission of this program is to improve the quality of life of residents with chronic lung disease.
Wii Habilitation is a new program that allows the resident to have fun while being involved in therapy services. We have an on-going program including Wii sports and Wii Fit.
The resident and family are important members of the team, and are directly involved with the establishment of a personalized treatment program that meets each resident's needs and goals. The rehabilitation program is coordinated with all services offered at Silver Ridge Healthcare Center in order to facilitate functional improvement and to promote self-sufficiency.
