- Palo Alto Sub-Acute and Rehabilitation Center staff do frequent Interdisciplinary Rounds where our Nurses, Rehabilitation Staff, Care Coordinators, Discharge Planners, and others carefully assess and plan each residents individualized care plan. We’ve found this clinical team approach an exceptional way to ensure that both our short-term rehabilitation residents and our long-term residents (and their loved ones) are assured of receiving the finest level of care anywhere!

- Palo Alto Sub-Acute and Rehabilitation Center prides itself on being a teaching facility for Certified Nurse Assistants (CNAs). After a careful interviewing process, we hire and train our very own CNAs to ensure they are prepared with the clinical skills and our philosophy of quality care as they prepare to enter the field of nursing. Many of our CNAs have gone on to become LVNs and RNs within our facilities. If you are inquiring at other Skilled Nursing & Rehabilitation centers please be sure to ask them if they are a teaching site for CNAs – it does make a difference!

Palo Alto Sub Acute and Rehabilitation has been recognized and is proud to have received the Covenant Care Clinical Excellence award consistently for the past eight years.
To qualify for the Award, a facility must meet several clinical standards of excellence.

Palo Alto exceeded all clinical management standards in the following areas: chemical restraints, physical restraints, weight loss, and in-house acquired pressure ulcers.

Our state survey results are far better than state and national averages on a consistant basis.
