What about someone who is under or overweight?

Unique to Pacific Coast Manor is our full-time staff dietitian who assesses all new residents and monitors their weights on a monthly and if necessary, weekly basis. After initial assessment by the dietitian and with consultation from the physician, resident, family and if necessary the speech therapist, we customize a diet for the individual resident. With our computerized menu system and extensive alternative menu items we are able to provide a diet that meets each individual's needs. With 18 separate diets and four textures to choose from, we can get pretty close to perfect for each resident. For those needing extra help, we also can provide supplements, a variety of snacks and for very difficult cases, medication interventions to ensure weight loss is reversed or minimized. To ensure hydration, our activity team offers special drinks including smoothies between meals.

Are you a Medicare/Medicaid certified Nursing facility?

Yes, we are a 99- bed Medicare/Medicaid certified nursing facility and we also have contracts with a variety of private insurance managed care companies. To see if your plan is covered, please contact our business office for assistance.

Does the Department of Health certify this facility?

Yes. We are surveyed annually by the California Department of Health Services who re-certify us to participate in the Medicare and Medi-Cal programs and they also visit if there are complaints filed against the facility. These visits are always unannounced and every aspect of patient care is investigated. We have never been found to be providing "substandard quality of care" because we maintain rigorous standards and expectations for our employees with regard to each resident's care. The annual survey is available in the lobby for your review and we are happy to explain and answer any questions you may have regarding noted deficiencies.

What can I expect from nursing?

We have registered nurses (RNs) and licenses vocational nurses (LVNs) on duty seven days a week, on all shifts. Our nurses have to meet standards set by the State of California for licensure. The charge nurse supervises a staff of certified nursing assistants (CNAs) who help patients with bathing, dressing, and meals. In addition, the CNAs take daily vital signs; i.e., blood pressure, temperature, and pulse and report these to the charge nurse. Family members may also call the charge nurse at any time for a status report on a resident's medical condition. We are here to assist with the family's concerns as well as the residents.

How many days per week is therapy?

Physical, Occupational and Speech therapy is provided up to seven days per week depending upon the resident. All therapies require a physician's order and in the case of an HMO, therapy must also have authorization for treatment from the HMO's case manager or utilization manager. The amount of therapy received by a resident is dependent on how much progress the patient makes and therapists need to document significant progress to keep a resident on a caseload.

Who does the patient's laundry?

Our facility offers laundry services for a nominal charge or a family may elect to do a resident's laundry at home.

How much clothing should be provided for each resident?

We would like families to provide at least seven days of clothing including 2-3 warm sweaters, slippers, and a robe if possible. Clothing should be comfortable and easy to put on and take off. On admission the staff will provide you with a permanent marker and assist you in marking the clothing with the resident's name. Each article of clothing, and any personal items, will be listed on an inventory sheet for reference.

What are the visiting hours?

Visiting hours are 24 hours per day because we understand that most of us work and have difficulty visiting during the daytime. We ask that families use their best judgment in regard to the length and time of the visits. Please note we lock our doors after 8pm so please come to the side door by station 1 to be admitted.

Is there a hairdresser available?

Yes. The hair salon is open every Monday, 8:00 to 5:00 and our hairdresser sees both male and female residents. Hair care costs will show up on the monthly bill along with room charges or share of cost for our residents whose care is paid for by Medi-Cal.

How often does the doctor see the resident?

Physicians are required to see patients shortly after admission unless they have seen the resident in the hospital before discharge. The charge nurse communicates with the doctor at the time of admission for medication orders, dietary requirements, and other considerations. Doctors are required to make monthly visits for long-term care patients but may come in if a patient is experiencing a change of condition in medical status and physicians are available to us 24 hours per day by phone.

What if the patient needs a consultation for other services?

We contract with many ancillary service providers including (but not limited to) the following:
Audiology
Psychiatry
Psychology
Dentistry
Orthotics & Prosthetics
Podiatry
X-ray services
Optometry

Who do I contact you regarding insurance and billing questions?

Our business office manager is available during normal business hours to help you with most of your questions and concerns. If you have an HMO, you can also discuss any concerns you have with the case manager assigned to the resident.

Who do I see if I have dietary concerns?

We have a Registered Dietitian (RD) that is available to discuss resident preferences and needs with the resident and/or family. In addition, the dietitian also monitors each resident's dietary requirements, weight gain and/or loss, etc. In addition, we have a speech therapist available for consultation for any resident with swallowing difficulties. We ask family members to please check with the charge nurse concerning any possible diet restrictions before bringing in food from home.

Who will make transportation arrangements for residents who have appointments outside Pacific Coast Manor?

The nursing staff or the social worker will assist you in making transportation arrangements.

What types of activities do you provide?

We post a monthly calendar which shows each day's schedule of activities for our residents. These activities include (but are not limited to) the following: Bingo, Music, Arts & Crafts, Outings, Cooking classes, Adult Education, Movies, Furry Friends, Physical Fitness, Church Services, as well as activities with our local schools.

May we bring family pets in to visit?

We love pets and we insist that you bring them in often! Please be sure your best friend is on a leash.

Can family work with therapy and the resident?

Of Course! We encourage a holistic view of each resident and family is a major part of each resident's rehabilitation and care. Family participation is welcomed and family members will also learn therapy techniques that will prove to be invaluable upon discharge.

How do you communicate with staff to keep them updated?

Every 8 hours we have shift meetings to communicate changes and continuity of care. We have employee memos and monthly inservice training to keep our employees up to date.
