Case Management & Discharge Planning:

Two important components of our rehabilitation program that you will find only at Mission Skilled are on site discharge planning and case management.

Our Case Manager will make sure that during your stay at our facility that you will receive the appropriate level of therapy coverage by working closely with your physician, Medicare, HMO, or Insurance Company

Our Discharge Planner will work with you to insure a safe transition back to the community. This may include your need for additional therapy, nursing or special equipment to make your transition a smooth one.
