Los Altos Sub-Acute and Rehabilitation Center and its staff are dedicated to providing the highest quality Skilled Nursing and Residential care, in a comfortable, friendly and personal environment. The facility's size, (100 beds dedicated for long term and 52 beds dedicated to residents who need sub-acute and rehabilitation), and stability of our staff, gives us the opportunity and flexibility to provide a personal level of care not available in most Skilled Nursing Facilities. Since there are fewer residents assigned to each staff member, our staff can become better acquainted with each resident's needs as well as their likes and dislikes. The facility strives to create as much of a "home like" atmosphere as possible in an institutional setting.

Each staff member has the challenge and objective of making your visit to Los Altos Sub-Acute and Rehabilitation Center a pleasant one. The professional nursing staff, under direction of the attending physician, evaluates each resident and develops an overall plan of care. The plan of care is designed to meet the needs of the resident and helps him or her to achieve the highest level of independence and self-dignity throughout their stay with us. Above all, we pride ourselves on remembering that Los Altos Sub-Acute and Rehabilitation Center is more than a place of work for us, it is a home for our residents.

Along with our Nursing and Rehabilitation Programs, our residents enjoy the therapeutic effects of pet therapy and intergeneration programs. Our facility is the home for two poodles, a cat, as well as several birds and fish. With a Co Operative Children's Day Care on site, the Residents are able to work with children on projects as well as participate in intergenerational activities. Our friendly atmosphere makes our residents and visitors feel quite at home. All are welcome to visit. Meet our team of professionals who care for and care about our residents.
