We’ve found this clinical team approach an exceptional way to ensure that both our short-term rehabilitation residents and our long-term residents (and their loved ones) are assured of receiving the finest level of care anywhere!
- Los Altos Sub-Acute & Rehabilitation Center prides itself on being a teaching facility for Certified Nurse Assistants (CNAs). After a careful interviewing process, we hire and train our very own CNAs to ensure they are prepared with the clinical skills and our philosophy of quality care as they prepare to enter the field of nursing. Many of our CNAs have gone on to become LVNs and RNs here . If you are inquiring at other Skilled Nursing & Rehabilitation centers please be sure to ask them if they are a teaching site for CNAs – it does make a difference!

Los Altos Sub Acute and Rehabilitation received the 2007 Covenant Care Operation of Excellence Award and the 2007 Covenant Care Innovation award for our Valet Parking benefit to visitors.
Our Rehabilitation department specifically was awarded an Operational Award, and our Rehabilitation Director won the Chairman’s award which is the highest distinction.
To qualify for the Rehabilitation Operational Excellence Award, Los Altos exceeded the following standards of excellence including excellent service delivery resulting in:
- Positive patient care outcomes
- Initiating new programs and participation in those programs
- Positive feedback from customer and patient satisfaction surveys
- Excelling in community relations
