Choosing a health care and rehabilitation center for yourself or your loved one is one of the most important and difficult decisions you can make. You also want someone who is caring, knowledgeable, and accessible. You want someone willing to take the time to go over all of your options.

If you or a loved one are in need of health care, require advanced skilled nursing services and/or rehabilitation, then Lakeland is the choice for you. We provide excellent health care services in a homelike atmosphere, and are partnered with healthcare professionals who are dedicated to meeting your goals. You are at the first step in choosing a quality skilled nursing and rehabilitation center. As you move forward in your search enjoy your visit to our web site.

Lakeland Skilled Nursing and Rehabilitation is consistently rated among the best in the state according to the Indiana Department of Health's annual survey of all nursing facilities. This includes two recent annual surveys which deemed Lakeland as "Deficiency Free". Deficiency free means that no negative citations were given.

Every skilled nursing facility (SNF) is required to operate within a strict set of guidelines and regulations set forth by both state and federal governments. To determine compliance, every SNF is visited unannounced by a group of Department of Health personnel whose responsibility includes methodical and unbiased inspections of each facility; or if needed, extended investigations of activities that may demonstrate irregularities outside of "normal" SNF operations. Depending on the severity of deficiency findings from this visit, a SNF can be cited, penalized and required to comply, or to the extent of the law, may be ordered to cease, temporarily or permanently, its business operations. Findings for each facility are located in well marked public areas within a SNF to inspect findings. We are proud of our survey history and welcome the public to inquire.

When you are ready, please feel free to call us anytime to answer any questions, receive literature, or to set up a professional appointment. We are here for you 24-Hours a day, 7-days a week to meet your individual needs.

Lakeland's Homelike Atmosphere
To make the nursing home as home-like as possible is of utmost importance at Lakeland Skilled Nursing and Rehab Center. At the same time we must work within the restrictions of the physical plant of the building. We encourage Residents and their families to bring items that help make the environment as comfortable and pleasant as possible while not infringing on the rights of other Residents. Whether the Resident would like a phone or some of their own furniture, we will always work with them and their family to accommodate their needs. Lakeland is also proud to announce the addition of "open-choice" breakfast. For those early riser's, continental breakfast is now available beginning at 6:30 a.m., and a traditional buffet breakfast is available from 7 a.m. through 9 a.m.

What others have to say about us::

"Dear Lakeland Staff; How can I put into words how much your care and support meant to my father and myself? When I first brought my dad into a nursing home I was so upset and I worried about whether I was making the right decision. I cannot believe how much like family you all became in such a short time. Your staff became so emotionally and personally involved that I knew he was getting the best possible care…….."

"Attn: Mary Moore, Exec. Director; Just a little note to thank you and your staff for the many courtesies extended me while camping out in cell 111 or perhaps a 'royal suite' is a better description. I learned some new stuff….such as dressing the old bod while sitting down. You truly are doing God's work and I bless your souls for that. They are wonderful boys and girls and strong as all get out! I'm doing quite well at my little farm. Regards to your Dietician & serving mob---they do a wonderful job. Love to all, Sam"

"LAKELAND GAVE A SECOND CHANCE ON LIFE
To the editor: A new year! Time to count the blessings of the old. My greatest blessing came as a second chance at life where I became acquainted with Lakeland Nursing Home & Rehab. Lakeland is an attractive building set back off the road with well-tended grounds. When you enter as a resident you are greeted with smiles and friendly faces. Inside the building is clean and is always being swept, scrubbed, etc. So far, I've described many public facilities.
It's the family atmosphere that captures your senses. The caregivers, from the nurses to the high school trainees, are daily faced with problems that you and I would not care to handle, yet smiles remain. Each resident is addressed by his or her first name. Group activities, TV, books, tapes and games are available. Meals are very good (best soup in town). Physical, occupational and speech therapy are offered by highly qualified therapists. I was even able to witness how well an emergency was handled when the building lost its power for an hour with night fast approaching. Everyone acted quickly and professionally and a potential panic was averted.
Should you meet someone who tells you that he or she works at Lakeland, give that person an extra measure of respect. It is well deserved.
Sincerely, Barb Petersen, Angola"

"To: The Staff, Residents and Family of LNC:
From: The June class of Brown Mackie College Practical Nursing Students
Lakeland Nursing Center is not only a home to many, rehabilitation to most, but an educational environment as well. Each morning we walked in the door, we were greeted with a warm smile. The staff was informative, kind and appreciative. The residents were welcoming and very trusting. The experience we had here is really uncomparable to any other. Lakeland Nursing Center has put forth the knowledge for us to become the great nurses of tomorrow. And for that, there are no words that can express our sincere gratitude. Thank you for all you do!

"excerpt from a memo by Bob Levin (CEO of Covenant Care) to Mary Moore dated January 2007:
'For a facility to average 92% occupancy at a 47% Quality Mix (this means that they likely have a high turnover of Medicare patients) AND have a ZERO percent in all 4 clinical indicators (weight loss, pressure sores, chemical restraints and physical restraints) is extraordinary. My brief description does not do this accomplishment at Lakeland justice. We hold our facilities to the highest clinical standards and Lakeland just completely aced the test!' "
