At the time of admission, a written agreement shall be executed between the family and resident or responsible party (Power of Attorney). Resident's rights and responsibilities are reviewed, as well as an explanation of specific procedures that need to be followed at the facility.

We at La Jolla Nursing & Rehabilitation Center recognize that this admission process, from it's initial decision to final transfer and signing of forms, can be very traumatic for the resident as well as for the family members. We are here to give support as needed, answer questions, and give guidance during the transition period.

Following the initial assessment by nursing, other professionals from other departments (e. g., dietary, therapeutic recreation, social services) will visit the resident and family. An individualized plan of care will be formulated for the resident to assist in recognizing problems and setting goals; with approaches designed to meet the individual needs. Residents, individual care plans are reviewed every quarter. Family members are invited and encouraged to attend these meetings.

To ensure the safety of our residents, certain items cannot be kept at the bedside or brought in for use. Please check with us about these items.

All food items brought into the facility should conform to the resident's diet and must be stored in airtight containers.
