Grant Cuesta Nursing and Rehabilitation Center strives toward helping residents re-gain the independence necessary to return to their home or to an assisted living facility. Last year, we successfully discharged 150 residents back to their home and 22 to assisted living facilities. It is always a joyous event for us to see a resident going home from therapy being able to attend to themselves, or just being able to walk alone, or with minimum assistance or completely without assistance.
