Our therapy team is very proud of the successes that our resident’s have made during their stay here at Emerald Gardens. It is important to us that we highlight these successes because they help the resident truly understand how much functional Independence they have achieved from their date of admission to their date of discharge. We also like to share these success stories with the resident’s physician(s) and hospital discharge planner(s). This allow for a 360 degree approach to care of our residents by providing the follow through, after discharge, to the original care providers.
In the year 2007, our facility had 281 admissions and 103 of those residents were sent to a lower level of care. This includes discharges to home, assisted living facilities, and board & care facilities. This figure breaks down to 37% of our skilled therapy admissions were able to successfully return to live within the community. This is a true testament to the dedication and hard work of our talented therapy and nursing staff. We are looking forward to successfully sending 110 residents home in the year 2008.
Our Therapy Department is managed by an onsite Facility Rehab Director who coordinates the skilled services in Physical, Occupational, and Speech Therapy.

Our primary goal is always to
improve the resident's function to the highest level of
independence possible. We have state
of the art modalities that are the same utilized
by many professional and college sports teams. We believe
that our residents deserve the same million
dollar quality of care that million dollar athletes
receive.
Two major concerns of the elderly population are incontinence
and falls. Our therapy department is one of the few in the
area that has a continence improvement and a fall prevention
program to address these needs.

Our newest program, Wii-habilitation, utilizes the Nintendo Wii game system to improve the function of our residents. By playing a game of golf, tennis, baseball, boxing, or even bowling, our therapists can focus on a fun way to work on increasing the resident’s range of motion, strength, challenge balance, and build endurance. Whether they are standing or sitting, the residents are able to participate and benefit from this fun and enjoyable therapy program.

We offer our resident’s every opportunity to be able to return home safely and at their highest functional level. We have established a transitional living unit (TLU) to simulate, as closely as possible, the resident’s upcoming home situation. Once a resident has been identified as an appropriate candidate for the TLU, the therapy team determines the functional tasks that the resident will be performing based on the requirements necessary for a safe discharge to a lower level of care. If the resident is successful in the TLU, the interdisciplinary team may confidently suggest a discharge home, knowing that they have done everything they can do to assure this resident’s safety. If however, it is determined that the resident needs further assistance in any of the areas required upon discharging home, the team will recommend that the resident remain in the facility a little longer and the focus of treatment will be specific to those areas.
