The Social Services Designee is a highly trained individual who is committed to working with both the resident and family from the day of admission through discharge. Social Services works closely with the facility interdisciplinary team, the resident’s physician and family to insure that they stay at Downey Care Center is both comfortable and that the needs of the resident are met. From the day of admission, plans are put into place for the residents discharge as their condition improves so that they are ready to go home or to a lower level of care as soon as possible. Medical appointments are made through Social Services including transportation arrangements.

Social Services works constantly with insuring that the resident’s experience is secure and comfortable one. Families are encouraged to bring in a few of the resident’s personal items, which helps make their room more home like and recognizable.

Debbie Grant, our Social Service Designee, knows Downey Care Center. She has been a family member with us since 1984 working in various capacities before moving into Social Services in 2000. Debbie is a valued member of our leadership team and is pursuing her formal education in Social Work.
