SKILLED NURSING

Villa Springfield's skilled nursing facility provides both short-term rehabilitation and long-term care at the highest standards. We provide comprehensive skilled nursing care using state-of-the-art medical equipment and progressive treatment plans promoting quality care. An interdisciplinary care team made up of nursing staff, therapist(s), resident dietician, activities and social services develop a customized plan of care that addresses specific care needs and therapy goals necessary for the resident.

Some important points to consider when selecting a skilled nursing facility:

A nursing home, in Ohio, is a health care facility, licensed by the Ohio Department of Health, that offers a protective, therapeutic environment for those who need rehabilitative care or can no longer live independently because of chronic physical or mental conditions that require round the clock care. 

Older Ohioans now have more choices concerning how they live and the care and services they receive. Many options have emerged in the recent years to suit older adults. 

  • Be prepared. Don't wait until there is crisis. Visit several different providers to compare services.
  • Be realistic about health care needs. This is often difficult since one's inclination is to minimize problems as aging occurs. 
  • Be realistic about the environment. Beautiful surroundings and amenities are only part of the picture. Examine how the provider meets your personal interests, lifestyle preferences, religious and health care needs. 
  • Understand the limitations of the provider. In the scope of the services that can be legally provided.

Living Arrangements: Semi-private or private rooms 

Payment: Traditional Medicare meets the cost of short term, rehabilitation in any Medicare participating facility after a qualified hospital stay, (you can always ask the Social Worker or Admissions Co-Coordinator about the "qualified stay").

Some facilities also have a contract with different private insurances to cover the cost of short-term stay. (Again the Social Worker or the Admission Co-Coordinator at that particular facility should be able to help you out) 

Medicaid meets the cost of ongoing care in most facilities for those with income and resources below certain levels.

Arrangement from the private funds could also be made for the cost of the care.


Factors to consider: 

  • Villa accepts Medicare and Medicaid and most all insurances and HMOs. 
  • Does the facility participate in the Medicare or Medicaid program? Participation in such programs can assist in meeting the cost of care. 
  • If you are enrolled in Medicare HMO, will any post-acute care following hospitalization be covered in the facility?
  • Are amenities (such as private rooms, individual televisions, private telephones) available and at what cost? 
  • Is there an on-staff physician if needed? Does the resident physician visit the facility? 
  • What activity programs and special events are held at the facility? What excursions are planned for those who are able to take advantage of them? What religious activities are offered? 
  • What is the facility's policy on "do not resuscitate" orders, advance directives, durable power of attorney and other end of life decisions? 
  • Do the residents appear comfortable, well groomed and involved in meaningful activities? 
  • Is the overall facility well maintained, pleasing, cheerful?. Does the staff seem friendly, caring and accommodating to residents and visitors?
  • What are the visiting provisions? Is there a private area for visits? What are the normal visiting Hours?

Rehabilitation

Our AFFIRMA Rehabilitation program includes Physical Therapy, Occupational Therapy and Speech Therapy services, as well as a pulmonary program for qualified residents. The AFFIRMA therapy team's target is to rehabilitate residents to their maximum potential; and prepare them for discharge to a lower level of care, or simply return back home to their previous living situation. In 2012 we sent home over 261 residents through our Homeward Bound Therapy Program. 

Villa Springfield believes in combining cutting edge technology with proven therapeutic programs to improve outcomes, decrease length of stay, and to get each of our residents to their highest practicable level of function and discharged to home whenever possible. Villa Springfield strives to provide our community with the highest quality of service with the most advanced equipment available. 

Villa Springfield offers a full team of therapists to assist in both inpatient and outpatient settings. We have physical, occupational, speech, and pulmonary therapists with experience in treating a variety of neurological, orthopedic, and medical conditions, including pain management, contracture management, and urinary incontinence management. Individual treatment plans are custom made for each resident to allow them to return to their maximum independence.

Our Homeward Bound programs are intensive rehabilitation services specifically designed for the purpose of safely returning residents back to their former living arrangements in the community. 

The Homeward Bound programs provide ongoing evaluation and treatment for those who want to increase their functional levels in: 

  • ADLs (Activities of Daily Living), including feeding, toileting, dressing, mobility, transfers, urinary incontinence, and more
  • Safe ambulating on various surfaces throughout the home and community
  • Increasing strength and endurance to be able to manage independently or at a lower level of assistance
  • Wound management, respiratory health, contracture management, stroke recovery, orthopedic-fractures recovery and much more

NURSING SERVICES:
24-hours a day, 7 days a week. We accept patients requiring IV therapy, Central-Line management, TPN, wound care and trachs in addition to other skilled services.

Villa Springfield Therapy Room

REHABILITATION SERVICES:
Physical therapy, occupational therapy, speech and pulmonary rehab therapy. 7-day a week therapy, outpatient therapy, Tai Chi, Wii Therapy and Pet Therapy 

SOCIAL SERVICES:
Assists residents and family members with adjustment to placement, discharge planning, for special concerns and needs. 

DIETARY SERVICES:
Individualized therapeutic diets, licensed dietician consultations available, special dietary needs. 

PROFESSIONAL SERVICES:
24-hour delivery pharmacy services; laboratory, podiatry, optometry and hospice needs. Transitional Living Unit: A TLU is a home-like apartment setting preparing our residents to return home. The area is complete with a bed, refrigerator, microwave and private bathroom. Prior to discharge, we utilize our TLU to ensure our residents a safe and successful move back home. The TLU compliments our Homeward Bound Program.

Skin Care Program 

Transdisciplinary Team Approach
The scope of practice of both physical and occupational therapists, includes the prevention, identification, alleviation and resolution of acute and chronic soft tissue injury. The physical testing, measurement, assessment and treatment these therapists offer, provide an invaluable dimension in the care of the wound patient including:

  • Designing or recommendation of the splints, orthotics, support surfaces 
  • Positioning and safety devices 
  • Providing staff and family education specific to the patient needs  

 The Physical Therapist can add unique treatments to the care plan. 

 These treatments include: 

  • Electro therapy

  • Short-wave diathermy 
  • Sharp debridement
