Skilled Nursing

Pacific Coast Manor provides both short-term rehabilitation and long-term care at the highest standards. We provide comprehensive skilled nursing care using state-of-the-art medical equipment and progressive treatment plans promoting quality care. 

An interdisciplinary care team made up of nursing staff, therapist(s), resident dietitian, activities and social services develop a customized plan of care that addresses specific care needs and therapy goals necessary for the resident.

A sample of the nursing services provided:

  • IV Therapy, continuous antibiotics, TPN/Enteral feedings, PICC line insertion, blood transfusions
  • Extensive wound care management and treatment including Wound Vacuum treatments
  • Diabetic teaching, monitoring and treatment
  • Monitoring of nutrition, hydration, bowel & bladder functions
  • Catheter care/changes - urethral or supra-pubic
  • Colostomy/Ileostomy care
  • Comprehensive continence management program

Pacific Coast Manor's Intravenous Therapies Program
There may come a time when a loved one requires treatment of fluids through the vein, also known as Intravenous or IV therapy. Some of the things that can be administered through IV's are medication, hydration, high protein, comfort medications etc. IV therapy can be used to treat infections, malnutrition, dehydration and electrolyte.

Pacific Coast Manor's Dementia Care Services
Anyone caring for a family member who has some level of dementia, whether from Alzheimer's or a stroke, knows how difficult it can be. The level of needs of the resident can vary from moment to moment and it can sometimes be a constant process of adapting. At Pacific Coast Manor, the staff is trained to identify and care for the special needs of those with dementia.

Pacific Coast Manor's Stroke Recovery Program
The type and severity of a stroke influences the care and services required by residents with this diagnosis. Even residents with a history of a stroke in their past may still exhibit the lasting effects of stroke. The team at Pacific Coast Manor strives to help residents recuperate to their highest possible level. The best outcome is for residents to be able to return home on their own, but sometimes, due to the nature of illnesses and other conditions they may need additional help in their home or may need continued care at the nursing facility.

Pacific Coast Manor's Wound Treatment Process
There is a variety of causes for different types of wounds that may present themselves. No matter what the cause or the type of wound, Pacific Coast Manor's team consisting of nurses, aides, wound nurse, physicians, pharmacy and laboratory all come together to help minimize pain, heal the wound and prevent recurrences. We also work very closely with the wound care center in Watsonville for those needing the more extensive services that they can offer.

Pacific Coast Manor's Hospice Coordination Services
Hospice care may be provided in a variety of settings chosen by the patient or family such as the home, RCFE/assisted living or in a nursing facility like Pacific Coast Manor. Regardless of the setting, hospice care seeks to offer support to the terminally ill patient so that their final days may be spent with dignity in a setting appropriate for their care. Hospice care also offers counsel to family members and staff during the terminal phase of an illness and through supportive bereavement services after death.

Pacific Coast Manor's General Rehabilitation and Recuperation Process
A person may experience weakness and general debilitation due to falls, illnesses such as flu or pneumonia, improper nutrition or a variety of other diagnoses. The Pacific Coast Manor team offers a variety of services to help restore the person to an improved level of functioning.

Pacific Coast Manor Nursing Center's Pre - and Post - Op Care
When someone experiences a surgery be it for a hip fracture, knee replacement, gallbladder etc, it can be quite debilitating and may require extra care that you are unable to receive at home, but may not necessitate further hospitalization. This is where Pacific Coast Manor can step in and be of great assistance.

Rehabilitation

Pacific Coast Manor is proud to present our Homeward Bound Program focusing on short term rehabilitation. We have a separate, distinct unit in our facility with Neurological, Respiratory, Physical and Orthopedic based programs. Our therapy team is made up of physical, occupational, speech therapists and restorative nurse assistants. Each patient's rehabilitation program is designed to address the specific needs of the individual with treatment times varying from thirty minutes to three hours on a daily basis up to seven days per week.

Pacific Coast Manor Therapy Area

Physical Therapy focuses on identification, prevention and rehabilitation of individuals with physical impairment and pain. Following evaluation, the physical therapist develops an individualized treatment program addressing the areas of lower extremity strength, balance, coordination, functional movement, ambulation, endurance and general mobility. The goal is to achieve safety in a variety of areas including:

  • Walking
  • Posture
  • Rising/Sitting
  • Standing
  • Pain Reduction
  • Patient Education
  • Post-Amputation Care

Occupational Therapy addresses activities of daily living skills. The Occupational Therapist assists the patients in achieving their highest level of functioning, and in maximizing independence by focusing on: 

  • Dressing\Grooming
  • Eating
  • Bathing/Toileting
  • Homemaking
  • Community Reintegration
  • Home Assessments and Modifications
  • Splinting
  • Adaptive Equipment

Speech/Language Therapists provide a continuum of services including assessment, education and treatment of patients with communication, cognition and swallowing problems. Treatment programs are directed towards improving communication skills in the area of comprehension and expression. Patients admitted to the facility that are receiving nutrition partially or exclusively via a gastrostomy tube are evaluated for their potential to return to solid foods as appropriate. Speech therapists work to restore abilities in:

  • Listening
  • Swallowing
  • Speaking
  • Thinking
  • Hearing

The Restorative Nursing Program, included in the room and board charge, focuses on our long-term care population to maximize the quality of life for each resident who require a specialized restorative program. The therapy team and nursing staff develop individualized treatment plans for resident care in the areas of:

  • Ambulation
  • Range of motion
  • Activities of daily living
  • Positioning and alignment
  • Skin integrity and wound management
  • Contracture management including whirlpool treatments
  • Bowel and bladder training
  • Rehabilitative dining
  • Strengthening and restraint reduction.
